The Radiant Light is spreading & we are growing! Radiant Hospice seeks a compassionate Hospice RN experienced with providing direct in-home care to patients approaching end of life. Hospice RN oversees 12-15 patients within their assigned territory and coordinates with the IDG team to ensure continuity of care. The Hospice RN oversees the hospice patient's care and will collaborate with the other members of the patient's care team, attending physician, and patients and their families to coordinate quality care. RNs evaluate a patient's physical condition and develop care plans that address any of the patient's medical needs or needs for social and spiritual services. Apart from providing comfort to patients, RNs are adept at counseling and guiding patient's families as well.
